Monitoring & reporting Failure to conduct regular monitoring of drinking water quality, as required by SDWA, or to submit monitoring results in a timely fashion to the primacy agency or EPA.
Public notification & other Failure to provide notification to the public as required by the Violations of Public Notification Rule and the Consumer Confidence Report Rule, and violations of other requirements such as those violations related to variances and exemptions.

Again, this was due to a miscommunication on returning to standard monitoring after the PWS they purchase water from had brought treatment online. Standard monitoring required that they take 20 samples. A total of 23 samples were taken (8 in June, 15 in August). All 23 samples were below the AL. The 90th percentile based upon all 23 samples was 0.0005 mg/L for lead and 0.08 mg/L for copper. The original 8 samples taken WITHIN the appropriate range had a 90th percentile of 0 mg/L for lead (no detects), and 0.11 mg/L for copper, both of which were far below the 90th percentile, and no sample exceeded the AL at any point between the original 8 samples, the 15 makeup samples, and the 23 samples combined.
